Geriatric Clinical Tests and Measurements by Judy Thackaberry

Objective tests and measures are emerging as the standard of care in Physical Therapy. Medicare regulations specify this as an expectation of documentation. There are a myriad of tests and measures out there. This lecture will present an overview of selected tests and measures appropriate to Physical Therapy for use with adults and geriatric patients with a variety of conditions.

COURSE OBJECTIVES

The learner will recognize the relationship between performing patient tests and measures and quality care.
The learner will understand the Medicare requirement for objective tests and measures.
The learner will have knowledge of tests and measures in the following clinical areas: endurance, cognition, gait, balance + fall risk, and strength/functional performance.

Judy earned her MPT degree from Notre Dame College, Manchester, NH in 2001. She achieved her specialty in Geriatrics in 2006 and Exercise Expert for the Aging Adult certificate in 2010. Judy was on the NHAPTA Board of Directors from 2003 to 2008 and has been an active member of the NHAPTA Continuing Education Committee since 2004. She has worked in a variety of settings including acute care, subacute care, and outpatient. She works as the lab assistant in the Geriatrics course at Franklin Pierce University, Concord, NH during the winter term. She currently works full time in homecare for Lake Sunapee Region Homecare and Hospice in New London, NH.

Understanding and Applying Intramuscular Manual Therapy (aka) Trigger Point Dry Needling by Steve Coppola, DPT

This course will help introduce Intramuscular Manual Therapy in terms of some theory, indications, contraindications and philosophies of techniques. We will go into some detail about trigger points and their characteristics. You will also see a demonstration of the techniques using volunteers from the class.

Course Objectives:
1.    Understand the characteristics� of trigger points in the human body.
2.    Introduce Dry Needling concept to trigger points
3.    Understand some theories of trigger point causes
4.    Understand indications and contraindications to using a dry needle.
5.    See a live demonstration of IMS and the twitch response

Steven M. Coppola, MS, PT, DPT

Steven is the founder of the Coppola Physical Therapy facilities.

He has his Doctor of Physical Therapy from Franklin Pierce University and obtained his B.S. in Exercise Physiology, minor in Psychology and Masters in Physical Therapy from the University of Massachusetts at Lowell, where he sits on the Advisory Board.

Steven was also the medical coordinator for the Independent Professional Baseball Team �Nashua Pride� for two and a half years.

Steven has over 16 years of orthopaedic, sports and workers� compensation physical therapy experience. He has strong manual skills including spinal manipulation, Intramuscular Manual Therapy and myofascial work. He utilizes his Exercise Physiology degree to combine functional exercise with his manual work. Steven is a former comedian and now a renowned speaker throughout the east coast.

Steven has taken completed both Level 1 and Level 2 on Intramuscular Manual Therapy (IMS), also known as Trigger Point Dry Needling, from Kinetacore and has taken other IMS courses from Myopain Seminars and has combined the philosophies to effectively treat many orthopedic diagnoses.

Friday Agenda

The first weekend in March is going to be an exciting time for physical therapists, physical therapist assistants and PT and PTA students in New England. Experts from across the country and the region will be on hand for three distinct events aimed at three different PT markets.

Private practice owners from across the region will meet on Friday afternoon to enhance their ability to compete in today's market. Workshops include "How to protect your reputation from online attacks," "Retaining Staff: An Approach to Coaching and Progressive Discipline," "HIPAA Audits - What You Need to Know," and "APTA's New England Regional Pilot Update."

Concurrently, the first Regional Student Conclave will take place all day Friday. Please consider sponsoring a student to attend this event.

The 2nd annual Regional Public Policy Advocacy Forum will anchor the weekend on Saturday from 8 am - 4 pm. We are expecting over 150 attendees who are all interested in participating in the health care debate in this country. As states continue to change both how health care is delivered and how to pay for the system, PTs, PTAs and PT & PTA students will be engaged in the process. On Saturday, March 3rd, elected officials, lobbyists, communications experts and others will join us to ensure that our PTs can make a real difference.

Regional Student Conclave

Our first-ever regional Student Conclave will open the weekend's activities on Friday, March 2nd. The student event will run from 8 am � 5 pm. Based on the National Student Conclave and a very successful Connecticut event which has drawn over 100 students, this year's regional event will prepare graduating students for their entry into the profession. We are holding professional preparation classes in the morning including utilizing social networks, interviewing and resume tips and specific courses on potential national and international opportunities. The afternoon will feature clinical sessions in a variety of areas, particularly women's health, neurology, the shoulder and pediatrics.

Upper Extremity Management post CVA: Integrating Evidence and Clinical Practice by Lynn Wong

Following a CVA, 80-90% of individuals present with some level of hemiparesis. Six months following an acute CVA, 65% of individuals are unable to use their affected UE in a functional manner. These statistics have led to ongoing research regarding the best means to rehabilitate the affected upper extremity post stroke. What is considered current best practice? How can this be integrated into today�s clinic environment?

Course Objectives: Following this course the attendee should be able to:
1) Demonstrate understanding of the causes of UE dysfunction following CVA.
2) Summarize recent advances in the literature regarding rehabilitation of the affected UE post CVA.
3) Understand and appropriately apply the major theories / practices of UE management post CVA

Lynn has been a practicing physical therapist for over 20 years in a variety of settings: entry level PT programs at UMass Lowell under Dr. Susan O�Sullivan and Simmons College, home health, acute rehab, acute care, skilled nursing facilities. She received her BS in physical therapy from Ithaca College, and her MS and t-DPT from MGH Institute of Health Professions. As part of her Masters� program, she completed a 3 month internship in PNF. With the ever rising costs of healthcare the incorporation of best evidence into practice is as important as ever. However the process of locating and appraising evidence prior to incorporation into practice can often be a daunting task for the working clinician. This course will review principles of evidence-based medicine including how to construct a well built clinical question, efficient methods of searching for and locating evidence, and critical appraisal.
Course Objectives:
Following this course, attendees will be able to:
�    Format a well built clinical question
�    Understand the hierarchy of evidence
�    Effectively perform critical appraisal

Sensory impairment is the result of insult to the nervous system which may lead to limitations in activity, functional disability, and increased risk of injury and/or secondary impairment.

Hyposensitivity (low response) can include anesthesia (total loss of sensation), partial or poor (low) registration of sensation, reduced sensory discrimination, and/or sensory neglect, while hypersensitivity (high response) can include sensory defensiveness, sensory avoidance, or paresthesia (altered or painful sensation). Intervention for sensory dysfunction entails a combination of working to facilitate recovery and working to help compensate for impairments that are or may become permanent. The decision to choose interventions focusing on recovery or compensation often depends on the diagnosis and medical status and may change over time depending on patient progress.

Interventions chosen should be based on evidenced based practice: a blend of current research evidence, therapist clinical judgment, and individual patient values. Sensory interventions for hyposensitivity and hypersensitivity to be discussed include: sensory integration, Wilbarger brushing program, functional training, sensory retraining, electrophysical agents, monochromatic infrared energy, compensatory strategies, vibration, intermittent pneumatic compression, and thermal modalities. (Interventions for paresthesia will not be addressed) Brief overviews of each intervention will be provided along with case examples to illustrate clinical applications. Discussion of available evidence and future research needs will be included.

OBJECTIVES:

Upon completion of this course, you will be able to:

1.Distinguish between normal and abnormal sensory function
2. Describe mechanisms of common pathologies related to sensory impairment
2. Recognize common signs and symptoms of sensory impairment
3. Formulate functional patient and family centered goals based on results of examination
4. Design an appropriate plan of care for hypersensitivity impairments
5. Design an appropriate plan of care for hyposensitivity impairments
6. Choose sensory interventions using current research and evidenced based practice
